To obtain your SST card upon completing the classes, you must submit the required documents listed below through one of the following methods: email them to safety@compu21.edu, send them via text message or WhatsApp to 718-708-0928, or visit our offices to drop off the documents.
- Your contact information (phone number)
- Your height and eye color (it will appear on your SST card)
- A Valid OSHA 30 Card, (both sides and not older than 5 years)
- A Valid ID, (NYS driver’s license or IDNYC card do not need proof of address)
- For out of state driver’s license, Passport or consular ID you will need to provide a proof of address. Example: Utility bill, bank statement, insurance statement, cell phone bill, hospital letter, apartment’s lease, among others (with your name and address and not older than 30 or 60 days or no date requirement depending on what kind of letter). - A picture of yourself as the sample, (with plenty of light, white background, no shadows, no hat, no glasses, no jacket) can be taken with your cellphone
